How to Register with the Portuguese National Health Service (SNS)

Access to Portugal's public healthcare system (SNS – Serviço Nacional de Saúde) is based on legal residency .

Once you become a resident of Portugal, you are entitled to register with your local health centre and obtain an SNS (Utente) number. Until residency is granted, you are expected to maintain private health insurance, particularly during visa stages.


Step 1: Ensure You Have Residency

You must first hold one of the following:

  • Portuguese residency card

  • Residency certificate (CRUE for EU citizens)

  • Official residency approval document

Without residency, you cannot fully register with the SNS (unless covered by an S1 arrangement — see below).


Step 2: Gather the Required Documents

To register at your local Health Centre, you will typically need:

  • Passport

  • Residency card or certificate

  • NIF (Portuguese tax number)

  • NISS (Social Security number)

  • Proof of address in Portugal (e.g., Certificate of Residence from your local Parish Council)

It's advisable to take original documents and copies.


Step 3: Visit Your Local Health Centre

Go to the Health Centre that corresponds to your residential address.

Request:

  • An SNS (User) number

  • Registration with a GP (family doctor)

In some areas, there may be waiting times for allocation of a dedicated GP due to demand. However, you will still be registered within the system.


Can You Apply at a Citizen Shop?

Some Loja do Cidadão or Espaço do Cidadão locations can now issue:

  • NIF (tax number)

  • NISS (Social Security number)

  • SNS (health number)

in one appointment. Availability varies by location and appointment booking is usually required.


If You Receive a UK or EU State Pension (S1 Form)

If you receive a state pension from the UK or another EU country, you will need:

  • An S1 form issued by the country paying your pension

You must:

  1. Obtain the S1 from the relevant authority in your home country

  2. Register the S1 with Portuguese Social Security

  3. Present confirmation at your local health centre

This allows Portugal to provide your healthcare while the pension-paying country covers the cost.


European Health Insurance Card (EHIC/GHIC)

Once a resident in Portugal:

  • If you are not receiving a UK or EU state pension, you should apply for a Portuguese EHIC for temporary travel within the EU.

  • If you receive a UK state pension, you should apply for a UK GHIC .

This ensures access to necessary healthcare when visiting other EU countries.


Online Registration (Limited Availability)

In some regions, it may be possible to begin the SNS registration process by email. Availability and procedures vary, so in-person registration remains the most reliable method.

You can find the relevant e-mail through a a quick Google Search (write Centro de Saude XXXXX (name of area) e-mail). For example, our documentation were registered to a Quarteira address when we applied, so we typed in Centro de Saude de Quarteira e-mail and an e-mail address came up. We actually were successful in e-mailing them a couple of times with all our relevant documents attached, and were given a family doctor and Utente number by return. This was the main mode during and immediately after Covid-19, we are not sure how successful this method is now.


Emergency Services

The national emergency number in Portugal is 112 (ambulance, police and fire). English-speaking operators are available.

The official SNS website is available in Portuguese and English.

A quick note, is we recommend you download the SNS App when you do get your SNS number as your doctor, vaccination and appointments will all be stored and visible here.
